Thank you for being a part of the Free to Bloom Collective: a group of parents dreaming up something different for our children. We are thrilled to offer your child many experimental activities they would not get in a traditional educational setting. Through time in nature, art, movement, and free play we allow your child to safely stretch their comfort zones and grow while in a safe and loving space. However, there are inherent risks involved in this type of education and setting and Free to Bloom, cannot be held liable for such accidents and injury.
By Signing Below I/We:
- I/We the parent(s) hereby give my/our approval for my/our child to participate in any/all activities during the school year. I/We will notify the tutor/staff of Free to Bloom Collective should I/We choose for my/our child to avoid specific activities for any reason.
- I/We assume all risks and hazards incidental to such participation with Free to Bloom Collective, including, but not limited to, transportation to and from group gatherings, field trips and activities, accidents, injury or death while at Free to Bloom Collective or while my/our child is in the care of tutors/volunteers of Free to Bloom Collective. I/We do hereby waive, absolve, indemnify and agree to hold harmless Free to Bloom Collective and their staff and guest participants and persons transporting/teaching my/our child.
- I/We understand medical insurance will NOT be provided by Free to Bloom Collective. and all medical costs incurred while in the care of Free to Bloom Collective is my/our responsibility.
- I/We agree in the event of an emergency that requires immediate action before I/We can be reached for a staff member of Free to Bloom Collective to use their best judgment in the treatment/care of my/our child, including but not limited to, CPR, bandaging, taking your child to the emergency room and administering medication to treat severe acute conditions such as allergic reaction, snake bite etc.
Informed Consent/General Release
PLEASE READ CAREFULLY AND SIGN BELOW TO INDICATE YOUR AGREEMENT. NOTE: THIS FORM INCLUDES A RELEASE OF LIABILITY.
Since participation in nature based schooling can be dangerous, Free to Bloom Collective requires that all participants (and their adult parent(s) or guardians) assume all risks associated with the school year by signing this general release. For and in consideration of my child being permitted to participate in Free to Bloom Collective activities, I hereby voluntarily release, discharge, waive and relinquish any and all claims or actions for damages for personal injury, permanent disability, death, or property damage which I or my child may have, or which may hereafter accrue to me or my child, as a result of my participation in Free to Bloom Collective activities during the school day and while I or my child are at the facility, on field trips, or off campus in a school related activity or for any other reason. This release is intended to discharge, in advance, Free to Bloom Collective, it’s representatives, volunteers, and tutors, and the guest teachers/participants from any and all liability arising out of or connected in any way with my child’s participation in the school years activities. I further understand that serious accidents occasionally occur and that participants occasionally sustain serious personal injuries, death or property damage as a consequence thereof. Knowing the risks, I have voluntarily applied for my child to attend Free to Bloom Collective and participate in the activities of the curriculum and thereby agree to assume those risks to release and hold harmless Free to Bloom Collective , its representatives, volunteers or tutors, or the owners or maintainers of any facility used by Free to Bloom Collective for educational or recreational activities, who (through negligence or carelessness) might otherwise be liable to me or to my child (or my heirs or assigns) for damages. I further understand and agree that this release, discharge, waiver, and assumption of risk is to be binding on my and my child’s heirs, executors, administrators, and assigns. I further agree to indemnify and to hold harmless Free to Bloom Collective, its representatives, volunteers and tutors, or the owners or maintainers of any facility used by Free to Bloom Collective for activities, for any loss, liability damage, cost or expense which may incur as a result of any injury or property damage I or my child may sustain while participating in the activity. I agree to comply with the program’s stated and customary terms and conditions for participation according to Free to Bloom Collective.
